Burke Myotherapy Policy and Procedures

 

We strongly believe that your time is valuable as is ours and we make every attempt to be ready for your appointment at the scheduled time. In our commitment to provide a unique and outstanding experience to all of our clients and out of consideration for our therapists’ time, we have adopted the following policies:

Upon arrival to the parking lot, we ask that you stay in your car and text our office you are here.  The door is locked when a session is in process. Time is allotted for intake and checkout but does sometimes take a few extra minutes.

Cancellation Policy and Fees

Your therapy times are reserved exclusively for you. We value your business and ask that you respect the office scheduling policies. Should you need to cancel or reschedule, please notify us at least 24 hours in advance you may use the reminder link or text or call.

Any cancellations with less than 24 hours of notice are subject to a cancellation fee of $50 ($30 for a half-hour session). If you are using a gift certificate for your service and do not give notice, you forfeit the gift certificate for the service, Clients who miss their appointments without giving any prior notification will be charged in full for the scheduled service. We recognize the time of our clients and staff is valuable and have implemented this policy for this reason. When you miss an appointment with us, our therapists not only lose your business, but also the potential business of other clients who could have scheduled an appointment for the same time.

After your 3rd failure to cancel will require a credit card or gift certificate to guarantee a reservation so please have your credit card and/or gift certificate ready when booking. If you are refuse this you will not be allowed to book or schedule.

 

LATE ARRIVAL POLICY

PATIENT APPOINTMENT TIMES ARE SCHEDULED TO GIVE BOTH PROVIDERS AND PATIENTS TIME TO ADDRESS INDIVIDUAL PATIENT’S NEEDS DURING THE VISIT. AS A COURTESY TO OTHER PATIENTS AND STAFF, APPOINTMENTS WILL BE AUTOMATICALLY CANCELLED 15 MINUTES AFTER SCHEDULED START TIME AND CHARGED ACCORDING TO OUR CANCELLATION POLICY. WE CANNOT GUARANTEE THAT LATE ARRIVALS WILL RECEIVE AN EXTENSION OF THEIR SCHEDULED APPOINTMENT. IN SPECIAL CASES AND WHEN OUR SCHEDULE WILL ALLOW, WE MAY BE ABLE TO ACCOMMODATE ANOTHER APPOINTMENT, THIS WILL BE AT OUR DISCRETION AND ONLY WITH PROPER ADVANCED NOTIFICATION OF YOUR LATE ARRIVAL.

 

 

INFORMED CONSENT
Prior to each massage session, the treatment plan will be discussed with you. At your first visit with me you will receive a copy of the massage therapy policies and will be asked to sign the consent stating that you have read the information, understand it, and agree to comply with the professional massage therapy policies and procedures. Clients who we have not seen for at least a year will also be asked to fill out this form.

 

PROFESSIONAL BOUNDARIES

  • Requests for sexual activity will not be tolerated, will be viewed as solicitation, and reported to the proper authorities under the guidelines of the massage therapy policies and procedures. The client will not be rescheduled if this occurs.
  • We do not massage anyone under the age of 18 unless the parent or guardian signs their form.

EXISTING AND NEW MEDICAL CONDITIONS

  • It is the responsibility of the client to keep the massage therapist informed of any medical treatment currently being taken, and to provide written permission from the physician, chiropractor, physical therapist, etc., that the massage may be continued.
  • The client must also keep the massage therapist informed of any changes in health conditions.

TREATING MINORS
If the child is younger than 18 years old, we will ask a parent or guardian to sign their form.

 

PRIVACY POLICY
Your massage therapy records are kept in the strictest confidence by this office. All client records are kept in a secure place and only those who need to see a client’s file for legitimate business or professional purposes have access to them. Your records will not be released to third parties, including health care providers and insurance companies without your written consent. Records may be surrendered if required by law.

 

CONDUCT
Inappropriate behavior will not be tolerated. Suggestive comments, sexual innuendos, touching of a massage therapist in an inappropriate way, will result in the termination of the massage session and the client will not be allowed to return for further massages. Our full fee will be expected.


 

 SICKNESS POLICY

Burke Myotherapy recognizes that both massage therapists and clients are vulnerable to infections and therefore ask clients to cancel appointments when they are feeling unwell. I will also inform you if I am unwell.

If you have any of the following contagious illnesses the massage session will be rescheduled:
COVID-19, Diarrhea, Vomiting, Fever, Chicken Pox, Measles, Mumps, Meningitis, Hepatitis A, Conjunctivitis, Rubella, Head Lice, Impetigo, Influenza, Meningococcal Disease, Polio, Ringworm of the body, feet or scalp, Scabies, Thrush, Whooping Cough and the Common Cold.
